Of course 90% of the time it's symbolic and it's metaphorical.
If somebody died every time we drew the Death card, there'd be nobody left.
And especially coming from that particular deck, I'd say it most likely signifies a symbolic transformation.
If I'd drawn the Death card from the traditional tarot, I might be a little more freaked out by it.
Accompanied by the Tower, "Transformation" is brought about by a serious shake-up...
Due to The Emperor, it's likely the shake-up deals with authority, and because of the Page of Swords, it would seem that it revolves around being watched/spied on from afar.
