He describes the endothelium as a "one cell thick layer that lines the inside of the 'pipes' in you that carry blood -- both the veins, and arteries."
9:00 he says that the endothelial layer keeps the veins and arteries healthy, and that it operates on/produces nitric oxide.
He says the nitrous oxide is produced from essentially a couple of amino acids and enzymes. He says it converts l-arginine and l-citrulline into nitric oxide.
9:46 he says nitric oxide only lives for half a second and therefore it's hard to study. He says the endothelial layer is lined with nitric oxide and it sends signals through the body, a lot them directing enzymatic reactions to environmental stressors. It's key in signaling other organs to 'get it together' and produce a reaction to whatever is going on environmentally.
Nitric oxide provides a physical component to what is called normotensive pressures... such as blood pressure. He says blood cells should never actually TOUCH the inside of a vein/artery, rather they should be held in suspension by the gaseous layer of the endothelium.
12:00 unhealthy endothelial layers do not produce nitric oxide, and this causes problems including hypertension/high blood pressure and likely, cancer, diabetes, etc. -- all the immunosuppressant conditions.
He says that it doesn't do any good to take l-arginine and l-citrulline as supplements if the endothelium is unhealthy.

I think this is such an important topic, because people who get put under for reasons that just aren't vital really are fucking up their bodies in ways that are totally regrettable. After having a wisdom tooth extracted, I experienced anesthesia amnesia that has permanently affected me (as far as I can tell) not to mention other issues I experienced from losing that tooth that NO ONE ever talks about because they never tie these things together.
